Tongue Diagnosis (She Zhen)
Tongue diagnosis is a visual assessment method of the tongue — body, color, shape, coating — enabling rapid identification of internal organ status, imbalance nature and pathology depth.
Presentation
Tongue diagnosis (舌诊) is, alongside pulse diagnosis, the most emblematic diagnostic tool in TCM. The tongue is considered a 'mirror of internal organs' due to its rich meridian connections, vascularization and innervation. It is analyzed by four parameters: body, color, shape and coating.
Core Principles
- Tongue mapping: tip = Heart/Lung; edges = Liver/Gallbladder; center = Spleen/Stomach; root = Kidney.
- Body color: Pale pink = normal; Red = Heat; Pale = Qi/Blood deficiency; Dark red/Purple = toxic Heat or Blood stagnation.
- Shape: Swollen = Dampness; Thin = Blood/Yin deficiency; Deviated = Wind/Stroke; Cracked = Yin deficiency.
- Coating: Thin white = normal; Thick white = Cold/Damp; Yellow = Heat; Greasy = Phlegm-Damp; Peeled = Yin deficiency.

Technique
Patient extends tongue naturally in good light. Observation 15–30 seconds maximum. Note body color, coating, shape, dental marks, sublingual veins.
