The constitutional terrain assessment is a fundamental naturopathic tool for evaluating each individual's unique biological constitution. The concept of "terrain" encompasses all hereditary, acquired and environmental characteristics that determine the organism's reactivity to aggressions. This notion, inherited from Claude Bernard and Antoine Béchamp, opposes the Pasteurian paradigm focused solely on the pathogenic agent: "The microbe is nothing, the terrain is everything."

In naturopathy, the terrain is evaluated using several complementary frameworks: Hippocratic temperaments, naturopathic constitutions and diatheses (pathological predispositions).

Hippocratic Temperaments

The Hippocratic classification identifies four fundamental temperaments. The sanguine type features a robust constitution, ruddy complexion, exuberant vitality and tendency toward blood overloads (plethora). The bilious type presents a dry, muscular physique, concentrated energy, tendency toward acidosis and hepatobiliary disorders. The nervous (or melancholic) type has a fine constitution with heightened nervous system sensitivity and tendency toward demineralization. The lymphatic (or phlegmatic) type shows a broad, soft constitution with slowed lymphatic circulation and tendency toward mucoid overloads.

Ménétrier's Diatheses

Dr. Jacques Ménétrier defined five diatheses based on trace elements. Diathesis 1 (allergic) corresponds to manganese and manifests as hyperreactivity, allergies and morning fatigue. Diathesis 2 (hyposthenic) corresponds to manganese-copper with progressive fatigue, recurrent infections and respiratory fragility. Diathesis 3 (dystonic) corresponds to manganese-cobalt with neurovegetative disorders, anxiety and circulatory problems. Diathesis 4 (anergic) corresponds to copper-gold-silver with profound fatigue, immune depression and premature aging. Diathesis 5 (maladaptation syndrome) corresponds to zinc-copper with endocrine disorders and adaptation difficulties.

Practical Applications

Identifying the terrain allows the naturopath to fully personalize their strategy. A "sanguine-plethoric" terrain will guide toward drainage cures and hypotoxic diet. A "nervous-demineralized" terrain will call for deep revitalization with remineralization. Terrain knowledge guides the choice of naturopathic techniques (nutrition, phytotherapy, hydrology, physical exercise), their intensity and timing.

Limitations and Complementarity

The terrain assessment is a guidance tool, not a medical diagnosis. It does not detect pathologies but identifies constitutional vulnerabilities. It must be complemented by regular medical follow-up. The proposed classifications are simplified models: most individuals present mixed terrains, with one or two dominant temperaments.
